IMMUNIZATIONS
In compliance with the updated law effective 9/1/01 of Chapter 14 of the New Jersey Sanitary Code, New Jersey Department of Health, schools need medical proof with dates that a child has received specified immunizations. New Jersey State law requires that all pupils have the following:
Preschool/Pre-Kindergarten
(This applies only to children attending pre-school or pre-Kindergarten)
Pneumococcal Conjugate Vaccine
Ages 2-11 months 2 doses
Ages 12 months through 59 months, one dose
Influenza Vaccine
Ages 6 months through 59 months, 1 dose annually to be given between September 1st and December 31st of each year.
Haemophilis B (Hib)
Ages 2-11 months - 2 doses
Ages 12-59 months - 1 dose
Grades Pre-school through Grade 8
Diphtheria/Pertussis
(whooping cough)/Tetanus
Children entering pre-school/pre-Kindergarten need a minimum of 4 doses.
Children 1-6 years old - 4 doses, with one dose given on or after the fourth birthday, or any 5 doses.
Children 7 or older - Series of 3 doses.
Hepatitis B All students
3 doses
Children 11-15, a 2-dose series is acceptable.
Polio
A minimum of 3 doses with 1 dose given on or after the 4th birthday, or any 4 doses provided that they are given 28 days a part.
Mumps
One dose of live mumps virus vaccine given on or after age one.
Rubella
(German Measles)
One dose of live rubella virus vaccine given on or after age one.
Measles Vaccine
Children over 15 months of age entering pre-school or pre-kindergarten need a minimum of one dose of a measles vaccine.
Children entering Kindergarten need two doses of a measles vaccine.
Varicella Vaccine
Children ages 19 months and older - one dose, administered no earlier than 4 days prior to 1st birthday. Laboratory evidence of immunity or physician or parental statement of varicella disease is also acceptable.
Grade 6
(In addition to the immunizations outlined above)
Tdap vaccine
(Tetanus, diphtheria, acellular pertussis)
September 1, 2009, children born on or after January 1, 1997, AND entering or attending Grade 6 - one dose of Tdap given no earlier than the 10th birthday AND five years have elapsed from the last DTP/Dtap or Td dose.
September 1, 2009, children born on or after January 1, 1997, AND transferred into a New Jersey school from another state or country one dose of Tdap, provided at least five years have elapsed from the last documented Td dose.
Meningococcal Vaccine
September 1, 2009, children born on or after January 1, 1997, AND entering or attending Grade 6 AND have reached the age of 11 one dose of a meningococcal-containing vaccine, such as the medically preferred meningococcal vaccine.
September 1, 2009, children born on or after January 1, 1997, AND transferred into a New Jersey school
