•  REQUIREMENTS FOR NEW STUDENTS

     

    Prior to entering school, all students must have the following:

    DPT/DtaP Four doses, the last dose being administered on or after your child's fourth birthday, OR any five doses.  Age seven or older, three doses.
    Polio Three doses, the last dose being administered on or after your child's fourth birthday, OR any four doses.  Age seven or older, three doses.  
    MMR One dose of MMR (Measles/Mumps/Rubella) on or after your child's first birthday.  
    Measles  A second dose of a Mealses containing vaccine (such as MMR, MR, or Measles only) with a time span of at least one month from the date of the first MMR, OR documentation of Measles immunity.  
    Hepatitis B  Three doses...if your child has not completed the Hepatitis B series, the series of three injections must be started prior to entering school and completed during that school year.  
    Varicella  One dose administered on or after your child's first birthday or written proof of chickenpox disease must be provided in writing.  
    Physical Exam 

    For students entering school for the first time, a current physical exam(within the last 365 days) performed by your child's physician is to be completed prior to entering school.  This exam should include both vision and hearing. 

    For students transferring into Voorhees Township School District, documentation of a previous school entry physical will be accepted.   

     

    In state(NJ) transfers, must provide proof of immunization at registration.  

    Out of state and out of country transfers will have a 30 day provisional status to obtain and submit proof of immunization.

    Students without the required immunizations, physical examination documentation or documentation of a previous school entry physical examination will NOT be permitted to remain in school.
