• In the event that your child needs to take medication during school hours, these criteria must be met for the school nurse to administer medication:
    1. A written order from the prescribing doctor stating the name of the medication, the dose, the time to be given, a start date and ending date and any side effects.  The order must be signed, dated and stamped by the prescribing doctor.
    2. A written note from the parent stating all the same information the doctor has ordered.
    3. The medication must be in its original container and clearly labeled with your child's name.
These laws and school board policy apply to ALL medications including prescription and over-the-counter medicines.
ALL medication must be brought to the nurse's office by a parent.  Students can not bring in the medication.
