Procedures to Register a New Student at Osage Elementary School

1.   Call Jill Locke at 856-428-2990 ext. 4165 for an appointment

2.   Complete Necessary Paperwork (Download from Website)

3.   Gather the Following Items to Bring for Registration:

a.   Proof of Residency

                                                            i.      Homeowner= Current Tax Bill and 2 Current Bills

                                                         ii.      Renter= Lease with student name included as occupant and 2 Current Bills

b.  Drivers License

c.    Childs Birth Certificate or Passport

d.   Childs Immunization Records

e.   Report Card and Test Scores from Previous School if Available

f.      Transfer Card from Previous School
