Sports Physicals

SPORTS PHYSICALS
In order to participate in an interscholastic or intramural sport, the following is required:
 
1. Every students who participates in a interscholastic and/or intramural sports program must have a current examination form on file in the nurse's office. 
AN EXAMINATION IS ONLY GOOD FOR 365 DAYS FROM THE ACTUAL SIGNED DATE OF EXAM.
  • Student must have a physical examination performed by his/her physician.  The examining physician must complete and sign Part B of the New Jersey Department of Education Athletic Pre-Participation Physical Examination Form. 
    Part B of this form needs to be completed only one time every year. 
    Use the form provided above.
  • Student and parent/guardian must complete and sign Part A (Health History Questionnaire) of the New Jersey Department of Education Athletic Pre-Participation Physical Examination Form. 
 
2. If your child already has a current examination form on file, you need to fill out a Health History Update for Sports and Parent Permission Forms.
  • Student and parent/guardian must complete and sign the Voorhees Middle School parent/guardian Interscholastic Athletic Team Participation Form. 
    Parent must fill out and sign the Health History Update Form
    This form must be completed for every sport in which the student wishes to participate.
