For your child to be able to receive medication during the school day, the following conditions must be met:
1. The medication must be in the original container
2. A note from the parent and physician must accompany the medication. Directions for dose, time, start date and termination date of medication needs to be specified. This applies to prescription medications as well as over-the-counter medications like Tylenol.
3. Your child's physician may fax the "doctor's note" to me at: 856-427- 0296 Attention: Sue Guerin, School Nurse.
